Course Description

In this Workshop you will discover how to engage in a balanced and broadened approach to risk assessment to support person-centered care that has led to an improved care experience for the older adult, their caregiver and the clinician.

Course Details

AHS Workshop Learning Objectives:

By the end of the workshop, participants will be able to 

  • Reflect on how their personal values and beliefs impact their approach to risk in clinical practice. 
  • Identify the key elements of a balanced and broadened approach to risk assessment
  • Understand the macro (societal), meso (sector and organizational) and micro (clinician, client) level factors that challenge the implementation of this approach and explore and possible solutions.
  • Apply a 4-step approach to risk assessment, management and communication to their clinical setting. 
  • Recognize barriers and facilitators to implementing this approach and understand how to overcome the barriers and leverage the facilitators. 
  • Integrate person-centered care principles into risk-related decision-making to support autonomy, dignity, and resilience
  • Explore ethical considerations in balancing protection with independence in the context of frailty
  • Collaborate across disciplines to support shared decision-making and holistic risk planning. 
  • Develop a personal action plan to apply workshop concepts in their own setting. 

Instructor Biography: 

Heather is a passionate healthcare leader who began rethinking risk in 2010 when she struggled with her approach to risk with her community-dwelling clients. With over 25 years of experience in geriatric care and team leadership, and a new found interest in knowledge translation, she now serves as Director of Programs and Partnerships at the Provincial Geriatrics Leadership Ontario Office—helping clinicians, teams and organizations deliver care that’s person-centered, evidenced based but most importantly grounded in what matters most to older adults.
